Step-by-Step Technique

Incision from lateral epicondyle to radial neck

Develop interval between anconeus and ECU

Supinate forearm to move PIN away from surgical field

Incise capsule and annular ligament

Inspect fracture and assess reparability

3+ fragments with >50% involvement - consider replacement

Reduce articular fragments

Provisional K-wire fixation

Headless compression screws (2.0mm) for large fragments

Mini-fragment plate in safe zone for complex patterns

Safe zone: 110° arc with forearm in neutral rotation

Check elbow ROM and PRUJ stability
