Step-by-Step Technique

Posterior portal - standard viewing portal (soft spot)

Anterior portal - rotator interval, working portal

Lateral portal - through deltoid for suture anchor placement

Diagnostic arthroscopy - assess tear size, retraction, tissue quality

Evaluate biceps - tenotomy/tenodesis if pathologic

Subacromial bursectomy for visualization

Release adhesions and mobilize cuff (interval slides)
