Step-by-Step Technique

Standard posterior, anterior, lateral portals

Additional posterolateral portal for anchor placement

Diagnostic arthroscopy - confirm irreparable tear

Assess articular surfaces - contraindicated if arthritic

Assess subscapularis integrity

Prepare glenoid footprint - superior glenoid at cuff insertion

Prepare greater tuberosity footprint

Graft options: dermal allograft (most common) or fascia lata autograft

Measure defect size - graft should be 5-7mm thick

Graft fixed to glenoid first - 2-3 anchors

Knotless anchors or suture bridge technique laterally

Side-to-side sutures to remaining infraspinatus posteriorly

Graft acts as superior restraint - prevents proximal migration
