A 45-year-old recreational runner presents with chronic medial heel pain for 8 months. Previous treatment for plantar fasciitis with stretching, orthotics, and two corticosteroid injections has failed. Physical examination reveals maximal tenderness at the medial calcaneal tuberosity with radiation along the medial heel.
