Crossover Toe Deformity

Lesser Toe Deformities

A 55-year-old woman presents with progressive dorsiflexion and medial deviation of her second toe, which now overlaps her hallux. She describes a 2-year history of pain in the second metatarsophalangeal joint with gradual worsening of the deformity. Examination shows the second toe sitting dorsal and medial to the hallux with the toe held in hyperextension. There is a positive vertical drawer test with greater than 50% translation.
