A 48-year-old overweight office worker presents with 6 months of right heel pain. The pain is worst with the first steps in the morning and after prolonged sitting, improving with walking but returning after prolonged activity. Examination reveals tenderness at the medial calcaneal tuberosity with pain reproduced by dorsiflexing the toes. There is limited ankle dorsiflexion with the knee extended.
