Forefoot Pathology
A 28-year-old female long-distance runner presents with 6 months of pain beneath the first metatarsal head. Pain is worse with running and push-off activities. Examination reveals tenderness over the tibial (medial) sesamoid with pain on passive hallux dorsiflexion. Plain radiographs show fragmentation of the tibial sesamoid with sclerotic margins.
