Forefoot Disorders
A 28-year-old ballet dancer presents with chronic pain under the first metatarsal head, worse with activities requiring push-off such as relevé. She localizes the pain to the plantar medial aspect. Examination reveals point tenderness directly over the tibial sesamoid with pain on forced dorsiflexion of the hallux. Radiographs show irregularity of the tibial sesamoid with possible fragmentation. MRI reveals bone marrow edema within the tibial sesamoid. Regarding sesamoid disorders of the hallux:
