Nerve Compression
A 45-year-old office worker presents with numbness in the small and ring fingers for 6 months. She notes symptoms worsen when using the phone or sleeping with elbows bent. Examination reveals positive Tinel's at the elbow, positive elbow flexion test, weakness of first dorsal interosseous and finger abduction, and subtle hypothenar wasting.
