A 42-year-old female manual laborer presents with 6 months of lateral forearm pain that radiates distally. The pain is worse with gripping and forearm rotation. She has tenderness 5cm distal to the lateral epicondyle, and pain is reproduced by resisted supination with the elbow extended. Wrist and finger extension strength is normal.
