An 82-year-old woman presents after a mechanical fall. She has severe upper neck pain and holds her head with her hands. Neurological examination is normal. CT cervical spine shows a fracture at the base of the odontoid process (dens) with 4mm posterior displacement. There is no atlantoaxial instability on flexion-extension views.
