Spinal Infections
A 35-year-old migrant from an endemic region presents with 6 months of progressive back pain, night sweats, and weight loss. Examination reveals a gibbus deformity at T12-L1 with paraspinal fullness. MRI shows vertebral body destruction with relative disc preservation, a large paraspinal collection extending into the psoas, and epidural extension causing cord compression.
