A 22-year-old female swimmer presents with bilateral shoulder pain and a sensation of instability with overhead activities. She denies any significant traumatic injury. On examination, she has a Beighton score of 6/9, positive sulcus sign bilaterally (Grade II in neutral, Grade I in external rotation), and positive load-and-shift testing in anterior and posterior directions.
