A 24-year-old professional volleyball player presents with 6 months of anterior knee pain localized to the inferior pole of the patella. Pain is worst during jumping and squatting activities and has progressively worsened despite rest. Examination reveals point tenderness at the patellar tendon insertion, pain with resisted knee extension, and a positive decline squat test.
