A 58-year-old male manual worker presents with sudden onset anterior shoulder pain while lifting a heavy object 2 days ago. He felt a "pop" and noted immediate bruising down the anterior arm. Examination reveals a visible bulge in the mid-arm ("Popeye" deformity) with ecchymosis. He has mild weakness of elbow flexion and supination compared to the opposite side.
