Massive Rotator Cuff Tears - Irreparable Tears and Reconstruction

Rotator Cuff Pathology

A 62-year-old right-hand dominant retired carpenter presents with progressive right shoulder weakness and pain over 18 months. He has difficulty lifting objects above shoulder height and performing overhead activities. Examination reveals active forward flexion limited to 90 degrees, significant weakness of external rotation, positive hornblower's sign, and a positive lag sign. MRI shows a massive rotator cuff tear involving the supraspinatus and infraspinatus with Goutallier stage 3 fatty infiltration and retraction to the glenoid.
