Bosworth Fracture-Dislocation

Ankle Trauma

A 42-year-old male presents after a motorcycle accident with severe ankle pain and deformity. The foot is markedly externally rotated and there is visible puckering of the skin over the medial malleolus ("axilla sign"). Initial closed reduction attempts in the emergency department are unsuccessful. AP radiograph shows a high fibular fracture (Weber C pattern) with ankle dislocation.
