A 52-year-old woman with type 2 diabetes presents with progressive right shoulder pain and stiffness over 8 months. She now has difficulty with daily activities such as reaching behind her back and overhead. The pain is worse at night. Examination reveals global restriction of both active and passive range of motion, with external rotation most significantly limited. Radiographs are normal except for mild osteopenia.
