Rotator Cuff Arthropathy

Shoulder Disorders

A 72-year-old woman presents with progressive right shoulder pain and dysfunction over 5 years. She has significant weakness of elevation and external rotation. Examination reveals marked atrophy of the supraspinatus and infraspinatus, a positive lift-off test (subscapularis weakness), active elevation to only 60 degrees, and crepitus with motion. Radiographs show superior humeral head migration with acromiohumeral interval of 3mm, glenohumeral arthritis, acetabularization of the acromion, and femoralization of the humeral head.
