For the displaced intracapsular neck of femur fracture in the typical 80-year-old, what is your default — cemented or uncemented hemiarthroplasty? Registry and trial data (and NICE/BOAST) lean cemented for lower revision and periprosthetic fracture, but bone-cement implantation syndrome keeps the debate alive. What do you do, and how do you mitigate BCIS in the frail patient?
