  • HIGH-ENERGY TRAUMA - motor vehicle accidents, falls from height (greater than 2 meters), crush injuries - accounts for 70-80% of ankle dislocations. Mechanism: high force applied to ankle joint overcomes ligament and bone strength simultaneously
  • SEVERE ANKLE FRACTURE with dislocation - trimalleolar fractures (medial, lateral, posterior malleolus), pilon fractures (distal tibia joint surface), talar dislocations. 98% of ankle dislocations have associated fractures - pure ligamentous ankle dislocation extremely rare
  • SPORTS INJURY - severe ankle twisting/rotation injuries in contact sports (rugby, AFL, gridiron) or skiing. Usually combined with fracture
  • FALLS - particularly falls from height landing on inverted/everted ankle
  • PEDESTRIAN vs MOTOR VEHICLE - high-energy axial loading and rotational forces

  • Contact sport athletes - rugby, AFL, gridiron players at risk for severe ankle injuries
  • Previous ankle fracture or ligament injury - weakened ankle structures may dislocate with lower energy trauma
