Evidence brief
Surgical Correction of Hallux Valgus: Osteotomy Selection
Osteotomy selection based on deformity severity
Mild-moderate: distal osteotomies (chevron, Mitchell)
Moderate-severe: diaphyseal osteotomies (Scarf, Ludloff)
Severe: proximal osteotomies or Lapidus
Patient satisfaction 80-90% with appropriate selection
Successful hallux valgus surgery requires matching the procedure to the deformity. IMA and HVA measurements guide osteotomy selection.
Know IMA/HVA thresholds. Mild (HVA<20, IMA<13): chevron. Moderate (HVA 20-40, IMA 13-16): Scarf or Ludloff. Severe (HVA>40, IMA>16): Lapidus or basal. Always assess DMAA, joint congruity, hypermobility. Discuss Scarf vs chevron controversy.
