Clinical Implications

This Swedish RCT provided level I evidence that fusion can be effective for chronic LBP, but patient selection remains controversial and critical for success.

Teaching Note

Know this landmark trial but also its criticisms: patient selection, comparison to 'usual care'. Discuss modern fusion indications: instability, deformity, failed conservative care >6 months. Avoid fusion for non-specific LBP. MRI Modic changes may predict response.
