A widened symphysis is NOT always traumatic. Peripartum symphysis pubis dysfunction can exceed 10mm, and osteitis pubis or septic arthritis of the symphysis causes pain with erosive (not separated) changes. In trauma, the discriminator from lateral compression is direction: open book OPENS the ring (external rotation), whereas lateral compression CLOSES it (internal rotation) - which is why a binder helps the former and may overcompress the latter.
