Step-by-Step: DIP Arthrodesis
Precise technique is required to prevent non-union.
1. Exposure
- Incision: H-shaped or T-shaped incision is best. The transverse limb should be distal to the DIP crease to allow good skin retraction.
- Tendon: Tenotomy of the terminal extensor tendon. It can be split longitudinally or transected (since it will be fused).
- Collaterals: Release collateral ligaments to allow full access.
2. Joint Preparation (The 'Cup and Cone')
- Principles: Maximize bone contact surface area.

The Standard Case

Counseling: For an index finger I would recommend fusion rather than arthroplasty. Unlike the PIP joint, the index DIP needs a stable post for precision pinch far more than it needs motion, and DIP arthroplasty data are limited to small low-quality series. Fusion gives reliable, durable pain relief with a stable pinch. I would only consider silicone arthroplasty in a low-demand patient who specifically values retaining tip motion in an ulnar digit, and counsel about a roughly 7% reoperation rate.
Position: 0-10 degrees of flexion (essentially straight). This allows the pulp to meet the thumb for precision pinch.
Risks: Non-union (10%), Infection (pin track), Nail deformity, Cold intolerance.
Mucous Cyst Complication
"A 50-year-old female presents with a burst mucous cyst on her middle finger. The area is red and discharging."
Acute Management: This is an open joint (compound). Requires washout (oral antibiotics usually insufficient if joint communicates). I would treat as potential septic arthritis - washout in theatre or minor ops.
Surgery: Once infection settles, she needs definitive excision + osteophytectomy to prevent recurrence.
Risk: Septic arthritis leading to osteomyelitis and destruction of the distal phalanx (requires amputation).
Fixation Choice and Nonunion
"A manual labourer underwent DIP arthrodesis with crossed K-wires 4 months ago. He has mild discomfort and the radiograph shows persistent lucency across the fusion site without hardware failure."
Fixation choice: Headless compression screws give interfragmentary compression, no exposed metalwork and earlier mobilisation, but cost more, are harder to remove and risk an iatrogenic phalangeal fracture from screw-to-bone size mismatch. K-wires are cheap, removable in clinic and forgiving in small or osteoporotic bone, but lack compression and carry a pin-tract infection risk. Both have comparable nonunion rates in series such as Stern's.
Assessment: Correlate symptoms with imaging. Examine for tenderness and motion at the fusion site. Persistent lucency at 4 months with pain on stressing suggests delayed/nonunion; CT can confirm if plain films are equivocal. Importantly, more than half of radiographic nonunions are clinically painless.
Management: If the union is stable and only mildly symptomatic, I would continue protection and observe, as a painless fibrous union may be acceptable. If it is painful and mobile, I would plan revision with thorough debridement to bleeding bone, bone graft if there is a defect, and compression fixation (headless screw), addressing any underlying infection first.
